Peace Officer Affirmation

I, the undersigned, am over the age of eighteen (18) and am fully competent to provide the information herein.

I swear and affirm to the following:

1. I am a peace officer in good standing with all applicable certifying or licensing agencies. My peace officer certification is valid and has not been suspended, revoked, or under review or investigation.
2. I am eligible to perform off-duty peace officer security services on a contractual basis.
3. I understand that, pursuant to the State laws in which I perform services individuals who are not actively certified as peace officers may be subject to civil and/or criminal penalties for acting as an off-duty peace officer.

I certify and acknowledge, under penalty of perjury, that I have read the foregoing and that the contents of this Affidavit are true and accurate to the best of my knowledge.
